Employee's hand caught in cotton fabric windup roll

Employee #1 was loading an Apex machine with a 12-in.-wide roll of cotton liner that was used to separate rubber strips in the manufacture of tire bands. The leading end was routed around guide bars and placed around the take-up roller. The start limit switch was wired back and the festoon weight that makes the bottom cutoff (limit) switch was secured by the leading end of a rubber flipper. Employee #1, who was wearing gloves, had his right foot on the bottom cutoff switch to jog the machine during this loading process. His left hand became caught and was rotated counterclockwise at the take-up roller. The force pulled his body to the left and onto the floor in a twisting motion so that he could not reach the bottom cutoff switch. Employee #1 sustained injuries to his left hand that required hospitalization.
