Employee's fingers amputated by V-belts

At approximately 7:00 a.m. on October 6, 1992, Employee #1, who was on call, responded to an alarm at a sewer lift station. He did not check to see if the pump was off or turn the pump off before he grabbed the belts to see if they were operating properly. The pump was still on and the Employee #1's four left fingers were immediately amputated by the two V-belts. He climbed approximately 18 feet up a ladder before using his two-way radio to call dispatch to call an ambulance. The supervisor, who was only a couple of blocks away, heard the conversation and promptly went to the site and bandaged the injured hand. Employee #1 was taken to the hospital, where two of his fingers were reattached.
