Employee's leg injured by falling tank section

Employee #1 was attempting to straighten out the weld on the slightly peaked interior seam of a 4 ft wide, 10 ft diameter tank section. It weighed about 2,000 lb and was hung approximately 1 ft from the ground on a lifting device held by a 10 ton overhead crane. When Employee #1 hit the seam with a large hammer, the tank section vibrated and came off the cradle device, struck the ground on edge, and tipped over. Employee #1 attempted to dive out of the way, but his right leg was struck by the overturning tank section. The leg was amputated two days later in the hospital.
