Investigation Summary
Employee #1 and a coworker were working on a trench that was being dug to place a moisture barrier around a chapel. They were instructed to clean dirt from the spoils pile that was observed in the bottom of the trench. While they were cleaning the trench, an observer noticed that the trench wall was beginning to fall. A warning was shouted and the coworker escaped. The wall fell on Employee #1, burying him.
