Investigation Summary
Employee #1 was using a shot blasting machine to work on the floors at a D-Mart in Salt Lake City, Utah. He turned off the machine, lifted the guard panel with his left hand, and inserted his right hand into the guarded opening. Employee #1's fingers contacted the revolving fan; several fingers were broken and his index finger was amputated. Although Employee #1 was properly trained, he did the opposite of what he had been instructed.
