Investigation Summary
EMPLOYEE #1 WAS OPERATING A LOOPER MACHINE, WHICH ATTACHES BARS AND LOOPS ONTO COPPER STARTER SHEETS FOR ELECTROLYTIC CELLS. FOR SOME REASON HE LEFT THE CONTROL STATION AND WENT TO THE RIVETING SECTION OF THE MACHINE. HE REACHED INTO THE POINT OF OPERATION AREA AND THE MACHINE CYCLED. THE THE RAMS CLOSED ONTO EMPLOYEE #1'S ARM, CRUSHING IT. OTHER OPERATORS OF THE LOOPER MACHINE ARE OF THE OPINION THAT EMPLOYEE #1 REACHED INTO THE MACHINE BECAUSE A LOOP WAS NOT IN PROPER POSITION, OR TO CLEAN A PHOTO CELL. IN EITHER CASE A PHOTO CELL WOULD BE COVERED AND THE MACHINE WOULD CYCLE. THE MACHINE WOULD ALSO HAVE TO BE LEFT IN THE AUTOMATIC MODE FOR IT TO CYCLE.
