Event: 12/29/1987
Five employees killed when motor ignites

Employees #1 through #5 were removing a core from a Peacekeeper stage 1 motor. The employees had popped the core more than 10 inches, then apparently went into the building to finish removing the core from the motor. It appears that the employees were using the hydraulic system at the facility, not at the remote control room, to remove the core, causing the motor to ignite. The burning happened so fast that the employees were unable to escape. Employees #1 through #5 were killed. Further investigation indicates that the employees' operating procedure was not in compliance with the shop traveler. Other crews were using the same procedures.
