EMPLOYEE #1 HAD COMPLETED LAYING A 30 INCH BY 8 INCH STORM DRAIN TILE WHEN THE SOUTH WALL OF THE 9 FOOT DEEP TRENCH CAVED IN. THE EMPLOYEE WAS PINNED AGAINST THE PIPE AND CRUSHED TO DEATH.
