HE WAS HANDING DOWN A 3 FOOT HOPPER SECTION WHEN HE FELL, HEAD FIRST, AND BROKE HIS NECK. THE CONCRETE FORM WAS NOT EQUIPPED WITH GUARDRAILS, NOR WAS HE USING ANY TYPE OF PERSONAL FALL PROTECTION.
