AT APPROXIMATELY 11:15 AM ON APRIL 25, 1991, EMPLOYEE #1 WAS SUPERVISING THE REMOVAL OF A TREE. A FALLING LIMB, ABOUT 5 INCHES THICK AND 7 FEET LONG, STRUCK HIM IN THE HEAD. HE DIED OF A FRACTURED SKULL.
