EMPLOYEE #1 WAS WORKING UNDER A VEHICLE REMOVING PARTS. EITHER THE VEHICLE SLIPPED OFF THE JACK OR THE JACK FAILED, ALLOWING THE VEHICLE TO FALL ONTO THE EMPLOYEE'S CHEST. HE WAS ASPHYXIATED BY THE WEIGHT OF THE VEHICLE.
