Employee killed when struck by falling metal stairway

Employee #1 was cleaning up cut firebrick fragments on the furnace floor level of a smelter. At the same time, three workers from another contractor were lowering a metal stairway, which was suspended above the area in which Employee #1 was working. The stairway was hooked to a chain fall and to a padeye. The weld on the padeye broke, dropping the stairway on top of Employee #1 and causing multiple injuries to his head, neck, chest and back, of which he died. Employee #1 and the workers had not been aware of each others' presence and no warning signs were posted.
