EMPLOYEE CRUSHED TO DEATH UNDER LOADER TRANSMISSION

EMPLOYEE #1 WAS INSTALLING A TRANSMISSION IN A FRONT END-LOADER. HE WAS USING A HYDRAULIC BOOM WINCH TO RAISE THE TRANSMISSION. A PORTABLE SLAVE UNIT WAS BEING USED TO CONTROL THE WINCH. FOR EASE OF ACCESS BY EMPLOYEE #1, THE CONTROL PANEL WAS ORIENTED TO HIM UPSIDE DOWN. HE ATTEMPTED TO ACTIVATE THE WINCH, BUT, BECAUSE OF THE UNFAMILIAR ORIENTATION OF THE CONTROL PANEL, ACTIVATED THE BOOM INSTEAD. THE TRANSMISSION LOWERED ONTO HIS CHEST, CRUSHING HIM.
