EMPLOYEE DROWNED WHEN HE FELL INTO CHANNEL AT WASTE PLANT

EMPLOYEE #1 WAS WORKING AT A WASTE WATER TREATMENT PLANT WASHING DOWN THE CHLORINE CONTACT CHANNEL WITH A GARDEN HOSE. HE FELL IN AND DROWNED. THE CORONER'S INITIAL REPORT INDICATED THAT EMPLOYEE #1 ALSO SUFFERED A HEART ATTACK. IT COULD NOT BE DETERMINED IF THE HEART ATTACK OCCURRED BEFORE OR AFTER THE FALL. HE HAD BEEN WORKING ALONE AND WAS FOUND IN 6 FEET OF WATER. THE CHANNEL IS 9 FEET DEEP. THE WALKING SURFACE IN THE AREA WAS ONLY 10 INCHES WIDE. NO GUARD RAILINGS, LIFE LINES, OR OTHER PERSONAL PROTECTIVE EQUIPMENT WERE USED. THIS JOB IS PERFORMED AT LEAST ONCE A DAY. THERE WERE NO FORMAL PROCEDURES OR GUIDELINES ADOPTED FOR THIS TASK.
