Investigation Summary
At 8:15 a.m. on March 11, 2015, an employee arrived at the site location to drop off a load of surplus machinery/equipment. The employee was mounting and un-mounting his vehicle in an effort to remove the straps from the load and was not visible to the fork truck operator. As the fork truck operator proceeded to lift a micro-separator from the bed of the flatbed trailer, he struck the employee, knocking him off the side of the truck. Unaware that the employee was knocked over the side, the fork truck operator continued lifting items from the bed of the truck. The load became unbalanced and was subsequently dropped, hitting the employee as he lay on the ground. The employee was killed from exsanguination due to crushing injuries of the trunk with lung, vascular and skeletal injury.
