EMPLOYEE CRUSHED TO DEATH IN BARREL MAKING MACHINE

ON FEB 6, 1987, SEVERAL EMPLOYEES WERE PERFORMING ADJUSTMENTS ON A SIX-STATION BARREL MAKING MACHINE. EMPLOYEE #1 WAS ATTEMPTING TO CYCLE THE MACHINE 1/6 OF A TURNTABLE REVOLUTION SO THAT THE NEXT BARREL HOLDING POST WOULD MOVE INTO THE ADJUSTMENT POSITION. WHEN HE REACHED FOR THE TRIP BUTTON OF THE MACHINE HE PLACED HIMSELF IN A HAZARDOUS POSITION BETWEEN ONE OF THE BARREL POSTS AND THE FIXED GLUING HEAD (ABOVE THE ADJACENT POST). WHEN THE MACHINE TRIPPED HE WAS CAUGHT IN THE CHEST AREA AND CRUSHED IN THE SCISSORS ACTION OF THE ROTATING POST AND THE FIXED HEAD IT MOVED UNDER.
