Employee killed when struck by recoiling machine

On March 27, 1991, Employee #1 was operating a recoiling machine. When he had completed a coil, he stopped the machine and was getting ready to band the coil for shipping. While he was standing next to the 22,000 lb coil, the drive shaft for the coiler broke. The load fell onto Employee #1, killing him. It was noted during the investigation that the failure could not have been foreseen.
