Investigation Summary
On August 3, 1989, Employee #1 was performing cleaning operations from a roof area which was located 13 feet above the ground level. During the cleaning operation Employee #1 encountered a vertical beam which he had to go around, while doing so he disconnected his lanyard from it's attachment point in order to reconnect it on the other side of the beam. After disconnecting the lanyard Employee #1 lost his balance and fell off the roof striking a platform on his way down. Employee #1 was wearing a lift belt with a 'd' ring. Employee #1 was provided with one lanyard.
