Investigation Summary
AN ELECTRIC UTILITY EMPLOYEE ENTERED A TRANSFORMER ROOM WITHOUT THE KNOWLEDGE OF HIS SUPERVISOR. THE EMPLOYEE CLIMBED TO THE TOP OF A TRANSFORMER (WITHOUT USING A LADDER). HE REMOVED THE GROUNDING CONDUCTORS AND DROPPED THEM TO THE GROUND. HE REMOVED HIS RUBBER INSULATING GLOVES AND DROPPED THEM TO THE GROUND NEAR THE CONDUCTORS. HE THEN MOVED TO THE OTHER SIDE OF THE TRANSFORMER AND BEGAN TO DESCEND. ON HIS WAY DOWN, HE CONTACTED A 34-KILOVOLT FEEDER LOCATED 22 INCHES FROM THE TRANSFORMER. THE EMPLOYEE WAS ELECTROCUTED.
