Employee killed when buried by trench cave-in

Employee #1 was laying pipe in a 60 ft long by 3 ft wide by 4 to 11 ft deep trench. The side walls were vertical and there was no protective system. Employee #1 was in the 11 ft deep section when a cave-in buried him under 6 ft of earth. He was killed. He had received no training in excavation safety.
