Investigation Summary
At about 9:30 a.m. on July 15, 1991, Employee #1 was positioned on a 47-inch square wood pallet that was elevated by the forks of a forklift truck about 16 to 18 feet high adjacent to a sidewalk light pole. The pallet did not have guardrails installed and Employee #1 was not wearing a safety belt. Employee #1 was in the process of replacing the light fixture and bulb on top of the light pole. The power source for the light was 220 volts. While working on the light he touched a wire and received a shock that caused him to lose his balance. He fell off the elevated pallet to the ground. Employee #1 died on August 12, 1991, at 12:05 p.m. as a result of the injuries he sustained.
