Investigation Summary
Employee #1 was asked to replace a light bulb in the spotlight outside and above the bay door. He obtained a 20-foot aluminum ladder from the warehouse and went outside. He raised the ladder to just under the light, which was 16 feet above the ground. The base of the ladder was 11 feet out from the wall. The supervisor was holding the base of the ladder when he thought Employee #1 said something to him. He put his left foot on the base of the right side rail and half stepped forward to try to hear Employee #1. At this time the ladder kicked out and Employee #1 fell 16 feet; he suffered two broken wrists and facial injuries. Employee #1 had received no training specific to ladder safety. The ladder in use had two bent rungs and one split rung.
