Employee's finger amputated in wrap machine

Employee #1 was one of four employees hand feeding the slots of Giblet Wrap Machine #2. The wrapping paper wrapped around the exit sealing wheels because of too much feed. Having been trained to clear simple jams because this machine frequently jammed, she, being in the lead first position next to the machine, stopped the machine. Employee #1 cleared the jam and restarted the machine, which wrapped again immediately. She again stopped the machine and cleared the jam. At restart, it wrapped a third time. Thinking she had stopped the machine, she reached into the exit approximately 9 in. from the sealing wheels, but the paper grabbed her finger, pulling it into the sealing wheels and removing the left index fingertip.
