Employee's fingers amputated in rotating knives

An adjacent pelletizer and machine had become jammed with plastic materials. Employee #1 had cleared the problem and was blowing off the rollers and knife rotor when the air nozzle became caught between the rubber and knurled rollers. Employee #1's left index and middle fingers and half of the thumb of his left hand were drawn into the 6 in. rotating knives, and were amputated. The employer had no written policy regarding the clearing of jammed machinery. The manufacturer's manual on site had specific procedures for clearing a jammed machine.
