Event: 11/07/1991
Employee completely buried by trench cave-in

Employee #1 was inspecting a 4 in. sewer pipe in a 2 ft wide by 12 ft long by 9 ft deep trench that was not shored or sloped and had no trench box or shield to protect the employee. Within seconds of entering the trench, a cave-in occurred, burying Employee #1 to the waist. A second cave-in occurred, and completely buried him. A coworker uncovered Employee #1's head and face and then summoned the emergency rescue squad. The fire department, emergency medical service, and other units soon arrived and took charge of the successful rescue efforts. Employee #1 was hospitalized.
