Investigation Summary

Investigation Nr: 14299044
Event: 12/28/1990
Employee's arm injured when caught by moving parts

Employee #1 was using a forklift truck to remove blocks from a conveyor unit when he noticed that the elevator of a concrete cubing machine was in the down position. Having dealt with this equipment before, he went over to the machine and reached into it to actuate an internal switch that would cause the packer plates to retract and the elevator to ascend. He actuated the switch and the packer plates began to retract. Before Employee #1 could remove his arm, it became caught between the stationary frame of the door opening and the moving parts of the equipment, probably the packer plate itself, though no one--including Employee #1--was really sure. He sustained a laceration to his arm near the elbow. The bleeding was controlled, and Employee #1 was transported to Peninsula General Hospital, where he received further medical attention.
