EMPLOYEE FRACTURED LEG DUE TO FALL DOWN CHUTE

Four employees were hired from a temporary agency to remove debris from a construction site. All four employees were working on the fourth floor gathering and throwing debris down a chute. The employees worked until 9:00 a.m. and took a break. Returning from the break, the employees noticed the guard rails around the debris chute opening had been removed. While throwing debris down the chute, Employee #1 lost his footing and fell into the chute opening, striking the roll-off container 40 ft below. He suffered a fractured right leg and lacerations on his head, and he was hospitalized.
