Employee injured in fall to roof when ladder slides

Employee #1 was using a metal extension ladder to travel down from the vulcanizing roof to the first floor roof. The ladder slid to the right, causing Employee #1 to fall 15 ft to pea gravel on the first floor roof. He suffered multiple injuries to his head, arm, knee, and ankle.
