Employee killed when struck by crane bridge

Employee #1 and coworkers had been assigned to complete a new installation of an electrical conduit and associated connections. They placed a portable extension ladder against a support column adjacent to the travel area of a radio operator type overhead traveling crane that was not tagged or locked out. Employee #1 was standing in an area approximately 16 ft 4 in. above the floor level with his right foot on the ladder. His left foot was on a section of pipe that was attached horizontally to the support column and was parallel with the crane track. He was attaching a section of electrical metallic tubing to a connection and had his back to the crane, which was traveling toward him. As Employee #1 was attempting to get both feet on the ladder to descend, the end of the crane bridge struck him above his left ear, crushing his head between the moving crane bridge and the support column on which the ladder was leaning. He was killed.
