Investigation Summary

Investigation Nr: 995522
Event: 10/20/1989
HIT BY DIRT- BEFORE TRENCH CAVE-IN

At approximately 10:15 a.m. on October 20, 1989, Employee #1 was in a trench that was approximately sixteen feet deep, nine feet wide at the top, thirty three feet in length at the bottom, and approximately sixty seven feet in length at the top. He was in the process of benching the grade with a shovel and laser. Employee #2 felt dirt hit his head. He was under the impression that a trench cave-in was eminent. He began to go towards the end of the trench for egress. Employee #1 had taken only a few steps when a dirt clog hit him at the junction of his neck and upper spine. The impact knocked him to the ground. The fire department and an ambulance were called to the scene. He was later treated for bruises and was released from the hospital approximately two hours later.
