Employee injured when excavation caves in

Employee #1 was breaking a structure's wall ties in an excavation that was approximately 15 ft wide by 14 ft deep. The side of the excavation collapsed and pinned Employee #1 against a wall form. Employee #1's left lower leg and ankle were fractured.
