Investigation Summary
At 6:15 p.m. on February 20, 2015, an employee was standing on top of an 8-foot step ladder, changing the light bulb in an exterior lamp that was mounted on the top of an eave. While the employee was standing on the ladder, the ladder became unbalanced and tipped over, causing the employee to fall approximately 8 feet to the ground. The employee struck his head on the pavement, causing trauma to the brain , and was killed.
