Investigation Summary
At approximately 11:45 a.m. on February 24, 1993, Employee #1 was tightening allen screws on an Al-Jon Inc. tire cutter while the machine was running and not locked out. While tightening the screws, Employee #1 apparently pushed the wrong control and actuated the cut assembly. Employee #1 was pinned, crushed, and killed in the machine.
