Employee cut above eyebrow by broken blade

Employee #1 was using an enclosed cut-off saw with a 26 inch blade to cut metal pipes to length. The blade parallel to the arbor broke and flew against a metal protective panel. The piece struck Employee #1 above the eyebrow. He sustained a cut that required nine stitches. Employee #1 lost no time from work. He had failed to use the limit switch, which caused the saw blade to remain down following the cut. The pressure of the pipe against the blade caused the blade to break.
