Investigation Summary

Investigation Nr: 170127179
Event: 07/29/1992
Employee's finger almost amputated by product bucket

Employee #1 was straightening candy packets for boxing. His hand was in the bucket that goes under the product counter. The counter dropped the products and shoved the employee's hand into the side of the frame where the sensor lights are located. The employee's fingers were caught in holes. The buckets moved, breaking and nearly amputating a finger. Employees and the supervisor were unaware of the holes. Apparently, the assistant supervisor told employees to work in the area on previous occasions when three to four lines operated.
