Employee's fingers amputated by shaper blade

At approximately 12:20 p.m. on January 20, 1988, Employee #1 was operating a Rockwell shaper, #1228304, to shape wood for chair frames. His left hand came in contact with the shaper blades, partially amputating his left index, middle, and ring fingers. It is unknown as to how Employee #1 came in contact with the shaper blade because the machine is adequately guarded. There were no witnesses.
