CONSTRUCTION WORKER KILLED WHEN HE FELL FROM A SCAFFOLD

AT APPROXIMATELY 2:40 PM ON FEBRUARY 24, 1988, EMPLOYEE #1 WAS PREPARING TO LAY THE LAST 25 BRICKS ON A CHIMNEY UNDER CONSTRUCTION. HE WAS WORKING FROM A JOB-MADE WOOD POLE SCAFFOLD BUILT AROUND THE CHIMNEY. THE PLATFORM WAS 27 FEET, 3 INCHES ABOVE THE GROUND AND HAD NO RAILING. THE EMPLOYEE HAD NO SAFETY BELT OR LANYARD, NOR WAS THERE A SAFETY NET OR CATCH PLATFORM. THE EMPLOYEE WAS LAST SEEN ALIVE WALKING AROUND THE SCAFFOLD AT ABOUT 2:30. HE WAS FOUND, DEAD, AT THE BASE OF THE SCAFFOLD AT ABOUT 2:40 PM.
