AT APPROXIMATELY 10:20 PM ON OCTOBER 12, 1987, EMPLOYEE #1 WAS ASSIGNED THE TASK OF "SCRAPMAN" (CLEANING UP SCRAP) AROUND THE PRESSES. AS HE STEPPED ONTO THE WORK PLATFORM OF PRESS C-4 TO TALK TO THE PRESS'S OPERATOR, HE PLACED HIS LEFT HAND ON THE PRESS HEEL BLOCK. THE PRESS OPERATOR DID NOT NOTICE EMPLOYEE #1'S PRESENCE AND ACTIVATED THE PRESS. WHEN THE PRESS CLOSED IT AMPUTATED EMPLOYEE #1'S MIDDLE FINGER, ABOVE THE SECOND JOINT, ON HIS LEFT HAND.
