Investigation Summary
At 1:00 p.m. on March 21, 1988, Employee #1 hooked a load to a side boom mounted on a caterpillar tractor. He jumped onto the moving tractor, or ran alongside of it, slipped and fell on the treads. He was crushed to death between the tractor treads and side-boom counterweight.
