Investigation Summary
At approximately 7:15 p.m. on or about February 21, 1992, an employee was operating a Mitsubishi industrial lift truck, serial #51130, in the accepted and customary manner as established by past and current practices governed within this business. Unbeknownst to the employee, when driving into warehouse #2's northwest entrance from the loading dock area of plant #1, he struck Employee #1. When the lift truck stopped, Employee #1 fell to the smooth dry concrete floor, injuring his head. He died without regaining consciousness approximately 3 hours 40 minutes later at Saint Joseph Medical Center of South Bend, IN. Both the Mishawaka Police Department and the Saint Joseph County Coroner Department investigated the accident.
