Four employees overexposed to carbon monoxide

Employees #1 and #2 were doing inventory in a 40 ft by 100 ft (4,200 sq. ft) walk-in refrigerator (box 7) with a 27 ft high ceiling. Employees #3 and #4 were unloading a cargo container, using propane-powered forklifts (Caterpillar TC400, serial #772697; Yale forklift, similar size) because the three electric forklifts were being used by other workers. The forklifts were driven in and out of box 7 for 1 1/2 to 2 hours. Employee #2 felt sick and went home after working 6:00 a.m. to 3:15 p.m. Employee #1 worked in box 7 from 2:30 to 5:00 p.m. and went home suffering nausea, headache, and dizziness. He vomited at home and went to the hospital after picking up Employee #2. Doctors administered 100 percent oxygen and kept them overnight in the hospital. Employee #3 had a headache and was nauseated and Employee #4 had a slight headache, but neitheremployee sought medical care. The employer has a rule against using gas-powered forklifts in refrigerators. Employee #1 was a supervisor but did not associate his illness with the presence of the forklift.
