Investigation Summary

Investigation Nr: 14305122
Event: 09/13/1989
FOUR EMPLOYEES HOSPITALIZED DUE TO SO2 EXPOSURE

Employees #1 through #4 were working in the digester area of a pulp mill. The four employees were overexposed to sulfur dioxide when the chemical started pouring out of digester Number 5 along with wood chips. The sulfur dioxide release occurred because the Number 5 acid fill valve on the Number 5 digester had not been closed and the lid to the digester was off allowing the acid to come past the valve, up through the digester and on to the floor. The company did not have an adequate lock out procedure in effect for electrically operated valves. The four employees all suffered from sulfur dioxide exposure of varying levels and were hospitalized.
