Event: 05/16/1989
Logging truck driver killed by falling logs

Employee #1, a logging truck driver, was releasing the binders from the logs on his truck and pulling off the wrapper. The load had not been restrained by an unloading machine and two logs fell from the truck, striking and killing him. Employee #1 should have waited for the load to be served by a front end loader before removing the wrapper. The sorting yard was narrow and the procedure for removing wrappers was badly defined and confusing. During the 2 1/2 weeks prior to this fatality, several loads had not been restrained by an unloading machine before the wrapper was taken off. No on-site supervisor was present at the sort yard to ensure that safety procedures were followed during off loading. In addition, the logs had been poorly loaded onto the truck.
