Investigation Summary
Employee #1 and a coworker were replacing burned out floodlight bulbs in fixtures that were mounted on the leading edge of a metal roof. Having finished their work on the west side of the building they proceeded over the roof to the east side. As they were moving, Employee #1 stepped through a fiberglass panel skylight and fell approximately 14 ft onto the concrete floor of the dock. He suffered craniocerebral trauma and was transported to the hospital, where he died the following day.
