Investigation Summary
On January 4, 2002, two employees were standing outside a trench box, inside an excavation, on top of a 108-inch-diameter concrete pipe. One employee was signaling a crane, as a section of pipe was being lowered into position in the trench box. The other employee, a helper, was standing behind the first employee. A section of the north wall of the trench collapsed striking the helper. The helper died of blunt force injuries and traumatic asphyxia.
