RUN OVER BY TRUCK
EMPLOYEE #1 WAS UNDER A TRAILER, GREASING THE LEFT REAR AXLE. THE TRUCK DRIVER GOT INTO THE TRUCK, STARTED THE ENGINE, RELEASED THE BRAKES, AND PROCEEDED FORWARD. EMPLOYEE #1 WAS RUN OVER AND KILLED. THE DRIVER WAS NOT AWARE THAT EMPLOYEE #1 WAS UNDER THE TRUCK.
