Investigation Summary
At approximately 7:30 a.m. on July 21, 1994, Employees #1 and #2 along with five other employees were preparing to lower a motor control center from the first floor to the boiler room level. They were using a Vermette lift to hold up the bottom end of the motor control center, while six employees including the foreman surrounded the top end and began to lift the unit. The unit tipped over and struck and pinned Employees #1 and #2 against the ledge at the south wall. Employee #1 received a fractured vertebra in his neck and Employee #2 received severe lacerations to his left leg and arm. Both employees were hospitalized.
