Investigation Summary

Investigation Nr: 14385025
Event: 07/15/1996
Employee dies of apparent heart attack

At approximately 10:00 a.m. on July 15, 1996, Employee #1, who had worked as a shipping clerk (order picker) for the last 24 years, reported to his job. He appeared pale and ill to his coworkers and was sweating profusely. As recommended, he went to the shipping office to rest until he felt better. A few minutes later he collapsed in the office and fell out of his chair. Paramedics were summoned, but they were unable to revive him. He was taken to the hospital, where an attending physician signed a death certificate indicating that the cause of death was heart-related. Employee #1 had a history of heart problems and had undergone triple bypass surgery a few years earlier. No autopsy was performed.
