Employee killed when run over and covered by load

While backing his belly dump semi-trailer truck into position, a truck driver backed over Employee #1. He then proceeded forward while dumping caliche, and partially covered Employee #1 with the dumped load. When the driver turned his rig around, he discovered he had run over and killed Employee #1.
