Employee killed when struck in head by toggle block

At approximately 9:30 a.m. on January 11, 1994, Employee #1 and a swamper had been assigned to lift a generator onto a trailer. The driver was attempting to raise the poles when the toggle block broke off from the latch (one of the cheek side plates broke off where the bolt goes through). The toggle block came through the rear window of the truck, striking Employee #1 on the right side of the head. He died of his injuries.
